"UR: Contact Form Builder" lets you create custom contact forms using multiple field types—text, email, radio buttons, checkboxes, dropdown menus, and paragraph text. Place each form anywhere on your store as an App Block. Customize fonts, colors, spacing, and button styles in the theme editor. Custom CSS is also supported.
