Is there a free plan, or do I have to pay from day one?
There is no permanent free plan — paid plans start at $12/month (Lite). However, every plan comes with a 14-day free trial, so you can test the app fully before being charged.
How long does setup take — can I do it without a developer?

All custom field data collected through the app is saved directly to Shopify's native customer metafields, so it remains in your Shopify admin even after uninstalling. On the theme side, the Theme App Extensions installation method leaves no leftover code in your theme when you remove the app — only shortcode installs would require manual cleanup.
